Description-
General Summary
Under general supervision and according to The Health Trust’s general policies and procedures, performs specialized duties to support the Medically Tailored Meal (MTM) Community Supports Program as funded through CalAIM. In collaboration with the MTM Team will perform specific duties to operationalize, document and improve the provision of Community Supports in Santa Clara County. This is a temporary, full time position 7/1/22 - 12/31/22.

Duties and Responsibilities
Refine the description and contents of CS grocery box offerings and provide nutrient analysis of boxes. 
Tailor grocery box offerings to different medical conditions
Develop a food ordering system or plan
Investigate into the possibility of outsourcing grocery boxes to a vendor
Assist with the development of Heart Healthy, Diabetic Friendly and Renal Menu options.
Develop 4-5 week cycle menus
Possibly develop recipes for the Renal menu (10-20 portions)
Nutrient analysis of meals and menus
Research finding a new food vendor for Medically Tailored Meals in Santa Clara County, either for all meal types or just renal meals.
Develop a tool to evaluate our MTM Program and client success.
Quantify savings on providing MTMs to clients by researching and putting a dollar value on the reduction in ER visits and hospitalizations.
Collaborate with the MTM team by performing other duties related to MTMs including, but not limited to:
Provide nutrition counseling sessions and assessments to MTM clients
Document nutrition therapy sessions and assessments 
Write up a meaningful MTM client stories
With Program Manager collaboration develop a compelling way to promote our MTM services
Uphold HIPAA office procedures and confidentiality

Experience and Training
Bachelor’s Degree in Nutritional Science from an accredited College or University or related field.
Certification as Registered Dietitian (RD). Must maintain a minimum 15 hours of annual continuing diabetes education.
Proficient in Microsoft Office products/software, Google applications
One year experience as a Registered Dietitian.
Bilingual in English/Spanish or English/Vietnamese preferred.

Minimum Knowledge and Ability
Possess strong clinical skills in medical nutritional therapeutics, including specialized nutritional support.
Ability to independently research topics, propose solutions and write up proposals.
Ability to work within a team environment and work independently, exercise sound judgment and assume responsibility.
Ability to complete documentation and written records in a timely manner.
Ability to concentrate and pay close attention to detail.
Ability to communicate effectively with diverse population groups and maintain a high standard of professional conduct.
Excellent interpersonal skills.
Effective writing and organization skills.

Working Conditions
Professional office environment, hybrid options available.
May perform repetitive motion activities and sitting for long periods of time.
Must have a car available for daily use, a valid California driver’s license and current automobile insurance
