Provides comprehensive case management and care coordination for a diverse panel of patients with identified needs spanning age, condition, and population variables.

“Coaches” (rather than “teaches”) patients to improve their health behavior to attain health-related goals, while working as part of a health care team (Providers, Nurses, Social Workers, Patients, Family, Caregivers, etc.)

Graduate from an accredited school of nursing.

Active, unrestricted licensure as a Registered Nurse in a state.

Current state’s driver’s license with the ability to travel to hospitals, skilled nursing facilities, patient’s homes, and other sites where patients receive care as needed.

Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.

Candidates must be able to perform all essential job functions with or without reasonable accommodation.

Certification in Geriatric Nursing or Geriatric Case Management.

5+ years of nursing experience, preferably with older patients.

Proficient in computer use, the Internet and health information technology.

Demonstrated excellent communication, interpersonal and organizational skills, with a flexible and creative approach to problem solving.

Proven ability to work effectively both independently as well as a member of an interdisciplinary team.
