Home Health Care Registered Nurse 

Home Health, RN

The Home Health nurse provides one-on-one care for patients in their homes. This includes bathing, dressing, wound care, medication management, and IV therapy. The Home Health nurse will provide care to patients who are elderly, terminally or chronically ill, disabled, or possibly recovering from an accident. They will need to be comfortable working independently and have strong clinical skills, such as administering shots and setting up and monitoring IVs. The Home Health nurse will also need to have a good bedside manner, which allows them to build trust and rapport with their patients.

Minimum Education/Experience:

  • ADN (Associate’s Degree in Nursing); BSN preferred (Bachelor’s of Science in Nursing).

  • NCLEX licensure per state requirements.

  • One-two years prior acute care experience or Home Health, Hospice, or Long Term Home Health Care program. 


  • Assess the physical, psychosocial, and social needs and conditions of the patient through physical exams and interviews to develop a plan of care. 

  • Collaborates with other members of the healthcare team to optimize patient outcomes, patient satisfaction, and healthcare goals. 

  • Complete and maintain accurate, complete and timely documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education intervention and evaluation according to Federal, Department of Health and The Joint Commission requirements.

  • Identify barriers to patient and patient’s family’s education and understanding of knowledge provided; engage patient and demonstrate caring behavior towards family and other caregivers.

  • Assists patients with daily living, which can involve positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals

  • Manages, plans, coordinates and executes the patient’s safe discharge from service. Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to regulatory requirements.
